Responsibilities: * Assist with activities of daily living: bathing, grooming, dressing, and meal preparation. * Monitor and record vital signs and report any changes to the healthcare team. * Provide companionship and emotional support to foster a positive environment. * Assist with mobility and transfers to ensure client safety. * Support memory care clients with reminders and engaging activities. * Collaborate with healthcare professionals to implement individualized care plans. * Maintain a clean, safe, and organized living environment for clients. * Follow all health and safety regulations during care delivery. Qualifications: * Previous experience in home care or healthcare setting required. * Familiarity with hospice care is a plus. * Ability to monitor vital signs and recognize changes in health status. * Strong communication skills (written and verbal) for interaction with clients, families, and healthcare teams. * Compassionate, patient, and dedicated to enhancing others' quality of life. * Must pass background checks in accordance with state requirements.
